If the paper passes the two referees and the Associate Editor, then it still has to go before the entire Editorial Board. The two referees are sent each others' comments after a final decision is reached. In cases of a revise and resubmit, the journal makes every effort to send it back to the original two reviewers. The referee process is blind, but I'm not sure whether the initial vetting from the Associate Editor is.
